The Transitional Outpatient Program (TOP) is an intermediate level of mental health care designed for adults 18 and older who require more intensive support than traditional outpatient therapy. TOP services include group therapy up to 10 times per week, weekly individual therapy, and psychiatric medications as needed for up to 6 months.

· TOP is 3-6 months long on average.

· Clients participate in 10 groups per week with 1 individual therapy session per week.

· Medicare clients receive one service per day (4 groups and 1 individual therapy session per week).

· Psychiatric evaluations and medication visits as needed.

· Clients will have access to referrals to supportive services offered by PMHC as well as community agencies.

· Graduation ceremony for successful program completion.

Target population: Adults, 18+ with mental health diagnoses. Services can be accessed through Walk-In Intake, referral from hospital, court system or internal referral from a therapist or prescriber.
