Documents Needed for Intake

  • Identification (Driver’s license or state ID) including social security number
  • Proof of Bucks County residence
  • Insurance cards/information
    • If you have no insurance, you will need proof of family income  (last year tax form, paystubs)
    • If you have no insurance your fees are based on your family income. If you have no income please bring a notarized letter stating you are not working and have no income.
  • Name/Phone numbers of emergency contact and Primary Care Physicians
  • Advance Directives, if applicable
  • Custody paperwork, if applicable

Insurance

We accept Medical Assistance, Medicaid, Medicare, and those who are uninsured. Limited Commercial Insurance accepted, copy of card required prior to intake.

Intake is the first step for all clients to begin services with PMHC’s Main Center programs (Outpatient therapy/psychiatry, Transitional Outpatient, Co-Occurring Program, and Acute Partial Hospitalization).

Clients will complete the demographic section and biopsychosocial section of the intake prior to being seen by a therapist or prescriber in order to assure that the appropriate matching is completed. The clinical intake determines what level of care the client is appropriate for based on information provided by the client and any paperwork they present with. Insurance verification is part of the intake process. Clients start the intake by presenting their ID and insurance and will move forward with the intake process only after insurance and identity has been verified.

Intakes that are required to be scheduled (hospital discharges and court referrals) will be scheduled at 8am Monday through Friday. Walk-in intakes will occur 8am – 3pm Monday through Thursday and 8am – 12pm on Fridays.

Target population: Adults, 18+ with both substance use disorders and mental health disorders, Children age 4 and up.
