Case
Manager Some duties include: Ø
Providing
assessment, planning supports, locating/linking/coordinating supports and
monitoring supports to individuals age 3 and above registered for Mental
Retardation services. Ø
Provide
Supports Coordination services including, but not limited to: Assessment of
needs, annual Plan development, locating appropriate services, coordinating
services and supports, monitoring services, referring to community and system
resources Ø
Meet
face to face with individuals and families to write an Annual Plan according
to regulations and guidelines. Qualifications/Requirements Requires
a Bachelor’s degree in human service or education field.
Experience
working with special needs clients a plus.
Valid
driver’s license. Approximately 50%
of work time will be spent in the office and 50% in the community performing
case management duties. _____________________________ Mobile
Therapist Full-time
position for an experienced mobile therapist. Some
duties include: Ø
Provides
community based, individual, and family therapy. Ø
Participates
with family in an ongoing manner, in the formulation, development,
implementation and monitoring of treatment planning. Qualifications/Requirements Master

person will coordinate the mental health and the physical health needs with
clients receiving services from Penndel Mental Health Center. Ø
Primary
responsibilities will include engagement of the client, coordination with the
primary care physician and development of a wellness plan. Qualifications
/ Requirements: Minimum
requirements include an RN degree and at least 1-year experience in either
Mental Health or Medical-Surgical Nursing. _____________________________ Licensed
Psychiatrist Some
duties include: Ø
Performs
psychiatric evaluations for the purpose of diagnosing the presence of mental
illness and treatment planning. Qualifications
/ Requirements: Experience
preferred in Outpatient services. ____________________________ Resident
Assistant Some
duties include: Ø
Select
constructive activities or situations suited to the resident’s specific
physical and emotional needs, and guides resident in activities, individually
or in groups, making adjustments in the program in accordance with
resident’s progress. Ø
Supervise
and assist residents to develop skills in the areas of personal health care,
nutrition, hygiene, and social skill development. Ø
Provide
individual instruction and assistance for residents when appropriate. Qualifications/Requirements Valid
Driver's License required. Some
Experience in MH Services preferred. High
