Transitional Outpatient

Transitional Outpatient Program (TOP) offers intensive outpatient services for those who need more than individual therapy once a week but less intensive treatment than inpatient or partial hospitalization. TOP include individual therapy once per week and group therapy between two and five days per week depending on the individual’s treatment plan.  Participants will receive medication management and be referred for additional services as warranted.

TOP serves Bucks County residents 18 and older who are diagnosed with Mental Illness or a combination of Mental Illness/Substance Abuse.  We accept Medical Assistance (MA/ Medicaid/ Magellan), Medicare and a variety of private insurances; insurance eligibility is determined on a case by case basis.

An individual may be referred to TOP by another treatment professional (psychiatrist, therapist or case manager).  Self referrals are also accepted for individuals currently receiving other mental health services at Penndel Mental Health Center. Once the referral is generated, TOP staff contacts the individual to schedule an initial appointment within a few days.  People who are not currently registered for services must first contact the Intake Unit to begin their services with the Agency.

TOP offers short term (4-6 months) intensive outpatient treatment.  The program uses an individualized, recovery-oriented, strengths based approach that is implemented through group and individual therapy.  TOP staff works on strengthening each individual’s support system to help them transition to less intensive Outpatient therapy and/or Medication Management treatment.  Services are offered up to 5 days a week with up to 4 services available each day.  Days usually consists of a combination of individual and group sessions based on topics such as: Self-Esteem; Anger Management; Addictions and Recovery; Recovering from Trauma; Communication Skills; Stress Management; Healthy Relationships; Grief and Loss; and Goal Setting.
