Health Connections

The Health Connections Program is designed to give individuals the extra support they sometimes need to understand and coordinate physical and behavioral health care. Members will work along with our Nurse Navigator to create and reach personal health and wellness goals in order to maintain a healthier life style. The Nurse Navigator will help educate members on their illness(es) and assist in learning how to manage it. Nurse Navigator will also act as an advocate for the member, while navigating the health care system and accessing services the member may need.

In order to participate in this program, individuals must be residents of Bucks County aged 18 and up. They must be diagnosed with serious mental illness in addition to a manageable physical health diagnosis and would benefit from the oversight and monitoring of a Nurse. Applicants with Medical Assistance and/or private insurance coverage may be eligible. Referrals for this program can be generated by treatment professionals (therapist, psychiatrist, Case Manager, Primary Care Physician)
